I don’t have a referral.

Under the Direct Access law in the State of Michigan, you may have an initial evaluation without a physician’s referral. However, not all insurances will reimburse you for the visit. Please call us and we will happily verify your eligibility for Direct Access at 616.847.1280.

We have four convenient locations for your physical therapy and athletic performance needs. Please clarify which location you prefer when you are scheduling an appointment.

We offer appointment reminders via cell phone text or email. We encourage you to set it up when you schedule so you receive a reminder for your first appointment.

There are a few required forms for you to fill out so we can best learn what’s going on and help you. We prefer that you complete them prior to your first visit. Fill out the online forms at least 24 hours prior to your first appointment. Click here to fill out your forms online.

If you have the following insurances, your insurance company requires an additional form to be filled out at your first appointment. If you have completed all the other required forms then please arrive five minutes early to complete this form so you can receive your full appointment time.

Bring Documentation

When you come to your initial appointment bring your driver’s license, insurance card(s), and referral if your doctor gave you a paper script.

Wear Loose Clothing

It’s best to wear loose, comfortable clothing that you can move in. Bring shorts if you have a lower extremity injury (knee, leg, ankle, foot) or wear layers for a back, shoulder or neck issue. Be changed and ready to go at your appointment time. Ask us where the changing rooms are located when you arrive.

During the initial evaluation, your therapist will listen to your history, perform a detailed movement assessment, and partner with you to design a treatment plan. The goal of the first visit is to create a plan that helps you reach your goal.

Your therapist will provide information about where you may not be moving optimally and what your future treatment will focus on. They will explain your prognosis and the timeline for recovery on this first visit. The assessment is usually 40 minutes, assuming it starts on time. You may be given exercises to do at home, as time permits.

Your therapist(s) will be checking in with you on your progress at each visit. Let them know how you feel like you are responding to PT.

After My First Appointment

Please be aware of your insurance benefits for Physical Therapy and any specific visit limits that may apply to you. It is your responsibility to check on your appointments and make sure that you have visits scheduled. We recommend that you have three weeks scheduled out at all times to ensure that you don’t accidentally forget to schedule. If you run out of appointments, you may need to work with another therapist until there is availability.

Feel free to ask the front desk about your scheduled visits before or after each appointment. Remember to clarify with your therapist(s) how much longer you may need treatment.

Payment is due prior to each visit. We accept check, cash, and credit or debit card.

You will receive an email from us in your first few weeks welcoming you to our monthly newsletter. We deliver special content to our email community that we don’t share anywhere else. You will also receive private updates and discounts on our special workshops and events.
